The first three months after Density or XERF — swelling, make-up, exercise and when to judge
Aftercare sheets often carry a long list of things to avoid. But many of those items are precautions for lasers or needle RF carried over unchanged to non-invasive RF. This column separates what is actually worth watching after Density or XERF from what has no evidence behind it.
The short answer
Non-invasive RF such as Density and XERF does not break the epidermis, so most aftercare is unnecessary. In measurements of 150 women, non-invasive RF produced no meaningful change in the skin barrier (transepidermal water loss), and two clinical trials that specified post-treatment instructions advised immediate return to normal activity and usual cosmetics, with zero cases of pigmentation. There are really two periods to watch after Density or XERF — burns appear within 72 hours and fat atrophy at one to four months (mean two months). Results are judged at two to three months.
What do you see after Density or XERF, and when?

Make-up, washing and work on the day of Density or XERF
Because non-invasive RF such as Density and XERF does not break the epidermis, clinical trials advised that washing and make-up can be done as usual from the same day. Microneedle RF, by contrast, raised transepidermal water loss on day 3 in a 20-person study, with recovery by days 5–7 — which is why aftercare rules differ even though both are called “RF”.

Is there evidence for avoiding exercise, saunas, alcohol or ice packs after Density or XERF?
We could not find any peer-reviewed study testing ice packs, or avoiding exercise, saunas, hot baths, alcohol, retinoids or massage, after non-invasive RF such as Density and XERF. The online sources for these items were mostly clinic information pages, and many were precautions carried over from laser or needle procedures.
In Density and XERF aftercare, “we could not find a study” does not mean harmful or harmless. Miso Clinic’s advice goes only as far as this: on the day itself, while redness and heat remain, it is more comfortable to put off things that heat the face further, such as a sauna.
Even if you are taking isotretinoin (acne medication), a consensus paper reviewing about 27 studies concluded that no waiting period is needed before procedures including RF such as Density and XERF.

Density and XERF aftercare: frequently asked questions
Can I wash my face and wear make-up on the day of Density?
Non-invasive RF such as Density does not break the epidermis, and clinical trials that specified post-treatment instructions advised immediate return to normal activity and usual cosmetics. If redness remains after Density, you can simply put off harsh products for a day.
When can I exercise or use a sauna after XERF?
We could not find any study testing the effect of avoiding exercise or saunas after non-invasive RF such as XERF. On the day itself, while redness and heat remain after XERF, it is more comfortable to put off a sauna that heats the face further.
How much downtime do Density and XERF have?
Most people return to normal activity the same day after Density or XERF. In studies, redness was 36.0% immediately, 16.7% at 72 hours and 3.9% at one month, and mild swelling lasts a day or two.
When are results judged after treatment?
Density and XERF results are judged two to three months later. In RF lifting research, clinical assessment was not significant at 4 weeks and became significant at 12 weeks.
Is there a period I need to watch closely after treatment?
