Do Density and XERF reduce cheek fat or a double chin?

A common explanation is that “XERF’s 2 MHz melts fat, so a double chin goes away”. This column separates where that explanation came from and what has actually been confirmed on human faces.

The short answer

As far as we could find, there are no human data showing that Density or XERF facial treatment reduces cheek or double-chin fat. The finding that XERF’s 2 MHz heats the fat layer broadly and deeply comes from computer simulation and porcine tissue, and most of its authors work for the manufacturer. In the United States a body-contouring device that claimed fat reduction as an indication had to submit clinical data, whereas the indication of XERF’s 510(k) was electrocoagulation and haemostasis. On the contrary, fat atrophy — excessive fat loss from RF heat, about 4 per 10,000 treatments — is classed as a side effect. The “lower-face thickness −0.89 mm” in the 16-person Density data is not a separate measurement of fat loss.

What are the data showing that XERF’s 2 MHz heats fat?

Of XERF’s two frequencies, 2 MHz heated the fat layer broadly and deeply in computer simulation and porcine skin histology, while 6.78 MHz heated locally along the fibrous septa (Lasers Med Sci 2025). The study was presented as the rationale for XERF’s two-frequency design; it did not measure whether fat decreased on human faces.

Does Density’s “reduced lower-face thickness” mean fat was lost?

The 16-person Density case series (J Cosmet Dermatol 2025) reported a mean 0.89 mm reduction in lower-face tissue thickness on 3D imaging, but this value did not measure skin, fat and fascia separately. These Density data cannot distinguish tissue tightening that changed the contour from a reduction in fat itself, and there is no control group.

Turning “Density reduced thickness” into “Density removed fat” is an extension the data do not support.

What would XERF need in order to claim fat reduction?

In the US 510(k) system, what decides whether clinical data must be submitted is not the procedure but the size of the indication being claimed, and XERF did not claim fat reduction. The body-contouring device K180709 included fat and circumference reduction in its indication and submitted two prospective clinical studies. XERF’s K251327, by contrast, gave electrocoagulation and haemostasis as its indication and submitted no clinical data.

In other words, the fact that “XERF has FDA 510(k) clearance” tells you nothing about fat reduction — and 510(k) clearance is not FDA approval. In Korea, XERF is approved by the Ministry of Food and Drug Safety (MFDS, the national medical device regulator) in the general-purpose electrosurgical unit class.

Why fat loss from Density or XERF heat is a side effect, not a benefit

In RF lifting, including Density and XERF, fat atrophy — fat loss from excess heat — appears one to four months (mean two months) later as one sunken spot, and is a side effect reported at about 4 per 10,000 treatments. Facial fat is also a structure that supports the contour, so Miso Clinic does not design Density or XERF treatment to put in extra heat to shrink cheek fat.

A 200-person ultrasound review found that sagging areas under the jaw had thinner skin and thicker superficial fat. Sagging and fat often occur together, so before Density or XERF we first separate which is the main cause.

Can XERF get rid of double-chin fat?

We could not find human data showing that XERF facial treatment reduces fat under the jaw. XERF’s 2 MHz fat-layer heating is a computer simulation and porcine tissue result. When sagging makes it look like a double chin, we consider XERF to tighten the sagging.

Does Density reduce cheek fat?

The 0.89 mm reduction in lower-face thickness in the 16-person Density data is not a measurement of fat alone, so it is not evidence that Density reduces cheek fat. Because of the risk of fat atrophy, Miso Clinic does not design Density treatment to add heat to shrink cheek fat.

Can the face hollow after RF lifting?

After RF lifting such as Density and XERF, fat atrophy rarely appears one to four months (mean two months) after treatment, reported at about 4 per 10,000 treatments. If one spot looks sunken, you should be seen early.

Has the FDA recognised XERF for fat reduction?

No. The indication of XERF’s 510(k) K251327 is electrocoagulation and haemostasis, no clinical data were submitted, and 510(k) clearance is not FDA approval. Another body-contouring device that claimed fat reduction as an indication did submit clinical data.
